Instructions

    • 1

Enter your most recent or current job first, unless the form says otherwise. Be sure to put your employer's official name, your official job title and any other information that the form requests.

    • 2

Move backwards through your career history and list each of the jobs on the form in the same way you entered your most recent or current job, making sure to keep the jobs in order with most recent first, second most recent next, then third and so on.

    • 3

Proofread your work history carefully. Make sure you have spelled everything correctly, filled it out completely and placed everything in the proper order. Remember, your work history form makes the first impression and could result in a job interview. You are competing against a lot of people. Don't give them a reason not to call you.

Tips & Warnings

  • Gather up all of the information you will need before beginning to fill out the form. You will need to know the names of all of your employers and supervisors, the exact dates you worked for the company, your job titles and your duties. You may also need to know your employers' addresses and phone numbers. Make sure you also know all of the correct spellings. You will have a much easier time filling out the form if you have this information pulled together before you begin.

  • List the skills and accomplishments that will most impress the prospective employer and leave off any job duties that would not matter.

  • Do not write "see resume" anywhere on the application. If a company wants you to fill out the work history form, then you must actually fill it out. Writing "see resume" will result in your application being automatically rejected.
