How to Create a Form in Microsoft Outlook
From creating an email message to inputing an appointment, many of the features in Microsoft Outlook are based on a form structure. Microsoft Office allows individuals to create a new form by modifying one of the many standard forms utilized by the program. There are a few easy steps involved in creating a form in Microsoft Outlook.

Create a new folder for storing the new form you'll be creating. Right-click on the folder list located in the left panel and select "Create New Folder." Type in the folder name in the designated box for the folder. Choose the appropriate folder contents from the drop-down menu. Choose a location for the newly created folder.

Select "Tools" from the Microsoft Outlook menu. Click "Forms," then choose "Design a Form." A window will appear showing a list of standard forms available for modification. Choose the standard form you want to modify. The selected standard form will open in the design view for modification.

Modify the standard form as desired to create your own form. To delete a box, click on the box until it's highlighted and then click on the "Delete" button to delete the box. Right-click on the field to change the title.

Add new information to the form by selecting from the options in the "Field Chooser" box. A window will appear giving a full range of new field options. To add a new field to the form, click on the desired field box and drag to the location where you want to place the new filed in the form. To add a field that is not listed, click on the "New" button. Type in the new field name, and select the type and format for the information.

Choose any controls that you want to add to the form. Right-click in the space where you want to add a control option. Choose "Control Toolbox" from the menu. Choose the control option you want to use, such as a selection box, from the options shown and drag it onto the form.

Save the form. Select "Forms" from the "Tools" menu. Choose "Publish Form." Choose "Browse" and then locate the folder you made in Step 2. Click on "OK" to select the folder location you designated. Type in the name for the form and select the "Publish" button.
