      Use the top section of your price order form for company information that customers can use to contact you to place orders and ask questions. Include your company name, phone number, fax number, email and website to provide customers with the greatest number of contact options. Include your company logo, if space permits, to further brand your company.

      Use the middle section of the form for customer information. Provide space for a name, address and phone number, as well as the order date and delivery instructions.

      Create a spreadsheet with vertical columns for order quantity, item description, unit price, and extension, or unit price multiplied by number of items ordered. If your company uses identifying numbers for its products, also include a column for the item number. Provide a sufficient number of horizontal lines for the order items, and a line underneath them for the subtotal. Add lines for sales tax, if applicable, and for any price adjustments such as discounts or credits from previous orders.
