How to Fill Out Customs Forms for Soldier Care Packages
If you want to send a care package to a soldier stationed overseas, you will have to fill out a customs form for each box before you can send it. This is easier than you might think, and a postal employee can help you with any part of the form. However, be sure to have a complete name and address, and be aware of the exact contents of the box before you begin to fill out the form.

Send your package flat rate priority mail for the most economical and time-efficient delivery, and choose customs form 2976-A. Before you pack your box, check with your soldier to see if there are any items that you can't send to their station.

Fill in the sender's name and complete address in the top address box--it should say "Sender" or "From."

Fill in the recipient's name and APO or FPO address in the second address box--it will say "Addressee" or "To," but do not include the country.

Below the recipient's address, a box marked "Detailed Description of Contents" must be filled in. List a brief description of the packages contents (i.e., personal hygiene items, cookies, drink mix, book, etc.).

Below this, check "Gift" (or the appropriate category) on the form. Additionally, find the box that requests information about what to do with package if it is undeliverable, and check the appropriate category in this box, as well.

Find the box that asks for the total value of the package contents in U.S. dollars and fill this in. Next to that box should be a box that asks for the weight of the package--have a postal employee weigh the package and fill that box in.

Sign and date the form, then give this to the postal employee. He or she can help you with any missing data or fields that may have been filled out incorrectly.
