How to Complete the Poisoned Water Supply Quest in Diablo
The Poisoned Water Supply quest is available only in the single-player mode of "Diablo." In any given game, you'll get either the Curse of King Leoric quest or the Poisoned Water Supply quest. The Poisoned Water Supply Quest is available if the water in the town's fountain has turned yellow. The reward is a unique ring, the Ring of Truth, which gives its wearer +10 life, -1 damage from enemies and +10 percent resist all.

Talk to Pepin the Healer, who will tell you that the town's water has been poisoned and is making the people sick, and that you must uncover the cause.

Go to Level 2 of the church and look for the entrance to the water supply sub-level, labeled "To a Dark Passage".

Enter the Dark Passage and kill all of the monsters. They are mostly Fallen, which are easy to kill, but there are a few Goat Men, who can be challenging for lower-level characters. Make sure you have plenty of potions. When all of the monsters are dead, a horn will sound and the water in the underground stream will turn from yellow to blue.

Return to Pepin. He will surmise that the monsters' mere presence caused the water to become poisoned and give you the Ring of Truth.

Tips & Warnings
The Ring of Truth is good for lower-level characters, but not very useful later on in the game. Keep an eye out for rings that give bonuses better-suited to your character.
It can be difficult to see over the cave walls in the Dark Passage, so "Diablo: Hellfire" players should use a search spell to locate all of their items.
