As a professional photographer, you should always carry copies of release forms.

It is important for a photographer to have a release form for models and location sites. These release forms relieve the photographer of responsibility from any injuries or consequences that could occur during the photo shoot and also offers provisions as to how the photographer can use the pictures in the future. Signed releases are especially important to have if the photographer plans to sell the photos for commercial use. A photographer should always have the model or location representative sign a release.

      Provide a space for either the photographer or the person signing the release form to enter that particular day's date. This can be included at the top of the form or at the bottom where space for the signature is provided. Some release forms have a space in both locations for the date.

      Provide a space for a person to write in his name, especially if the release is for a model. This initial space should have the person print his name rather than use his signature.

      Define the scope of the release. Most releases have a statement that reads as follows: "I hereby give permission to use my name and photographic likeness in all forms and media for advertising, trade, and any other lawful purposes."

      Include a space for your name as the photographer. If you are working as part of a company, its name should be included as well.

      Include a space for the person signing the form to sign his signature. If the release is for a property rather than a model, there should be a space available for the name of the location. For example, if a photo shoot is taking place at a restaurant, and the manager is signing on behalf of the restaurant, there should be a space for both the manager's name and the name of the restaurant.

  • If the person signing the form is under 18, then a legal guardian must also sign the form.
