How to Fill Out a Credit Dispute Form to Delete Items
As convenient as credit cards can be, unfortunately, sometimes you can run into trouble. Perhaps you received a charge that was much higher than you thought it would be, or got charged a signup fee for something you never signed up for. Maybe you just don't know where a charge came from at all. Whatever the situation, all credit card companies offer recourse by allowing you to register a credit dispute.

Gather all information related to the item or items on your credit card bill that you wish to have deleted. This includes receipts, email confirmations, and any correspondence with the company that charged you.

Download and print a credit dispute form from your credit card company's website. Most forms can be found in the "customer support" or equivalent section; some can even be filled out online, though you'll still need to send supporting evidence through the mail.

Include as much information as possible on the form. In addition to the required information identifying the charge, the key to disputing a charge is including all the details so that the credit card company can more easily investigate the fraudulent charge. Most forms have an "additional details" section or allow you to include a page for notes; do so, to make sure the credit card company is getting the whole story.

Locate the address to send the form to and all supporting evidence, and mail everything in one package. The address will be included on the form or on the Web page where the form is accessible.
