      Obtain Social Security Form 1199A. You can download and print the form from the internet or visit your nearest Social Security office to get a hard copy.

      Fill out the top section of form 1199A. You will need your claim or payroll ID number and the account number for your new account. If you plan to have funds directly deposited into a joint account, have the other account owner review the Special Notice to Joint Account Holders and sign in the appropriate box in Section 1. Remember to sign the top section when you are finished.

      Visit your new bank or financial institution. Ask a bank representative to fill out the lower section of Form 1199A. Make sure the representative signs and dates the form. Your bank should mail the form to the government office identified in Section 2.
