Write the main body of the personal statement. This step is identical for both types of personal statements.

Add a verification to a written personal statement made under oath. The verification reads:
"The undersigned hereby verifies on her oath that she prepared the above and foregoing personal statement and that it is true and correct to the best of her knowledge and belief."

Insert two signature lines at the bottom of a personal statement under oath. The first signature line is for you, the second for a notary public.

Sign the personal statement under oath in front of a notary public.

Insert one signature line with a space for a date on the simple personal statement (the personal statement not made under oath).

Keep in mind that when you sign a personal statement on your oath you are swearing that the contents are true and correct. If you provide false information in such a verified personal statement, you face the prospect of being charged with perjury.
