It is important to have the correct name associated with your Social Security Number to avoid delays in tax refunds and problems with your future Social Security benefits. There is a form that you can submit, along with specific required documentation, to your local Social Security office to correct the problem easily.

Place your driver's license, a U.S. passport or a state-issued identification in an envelope that you will take or mail to a Social Security office. These documents are required to prove your identity. Note that all documentation must be original. The Social Security office will not accept photocopies.

Add all documents related to the reason for the change required on your Social Security card, such as a marriage license, court papers for a legal name change, or proof of your divorce or annulment. In the case of a name change, you will need two additional documents to prove your identity. One in your old name, which can be expired and one in the new name, which must be current.

Print Social Security form SS-5 on standard 8 1/2" x 11" white paper and fill it out in black or blue ink. Place the completed form in the envelope.

Take the envelope with the application and all of your documents to a local Social Security office to apply for the name change on your Social Security card.
