How to Report Insurance Fraud in Texas

Insurance fraud involves willful misrepresentation by a policyholder, applicant or insurance company resulting in injury of some form. Insurance fraud schemes usually occur during one of these phases: the purchase of a policy, the sale of a policy, the use of a policy, or the approval of a policy. Insurance fraud, like other types of fraud, is a crime punishable by law in Texas. In fact, it is for this reason that the Texas Department of Insurance (TDI) has a Fraud Unit task force focused on reducing this crime.

Instructions

    Reporting Insurance Fraud in Texas

  1. Step 1

    Visit the homepage of TDI Fraud Unit online, and download a printable copy of Form FR028, Suspected Insurance Fraud Report (SIU) Form. You can find the link to this download in the Resources section of this article.

  2. Step 2

    Complete the printed Form FR028, making sure that you provide details about the following:
    - Information about the alleged fraudulent party;
    - Information about the fraud committed; and
    - Information about your injury resulting from the fraudulent scheme, such as the amount of money you lost.

    Be sure to include any supporting documents or evidence that you have.

  3. Step 3

    Mail your report along with any supporting documentation to the following address:

    Fraud Unit
    P.O. Box 149336
    Mail Code 109-3A
    Austin, TX 78714-9336

    Upon receipt of your report, TDI Fraud Unit will contact you within three business days.

Tips & Warnings
  • If you would like to file a report to TDI Fraud Unit as soon as possible, you also have the option of filing an online Suspected Insurance Fraud Report. See the Resources section for the link to this form. The main advantage of using this option is that TDI Fraud Unit downloads submitted online fraud reports every business day. However, note that you will not be able to attach any supporting evidence when using the online Suspected Insurance Fraud Report.
