How to Organize Medical Charts
Keeping proper medical records is crucial to the running of any medical office. A well-maintained record will easily tell you about a patient's history in the office and any outside physician office if consent has been given. It is very important to keep the records well organized to improve accessibility. You can implement a simple system to ensure continued organization of your office's medical records.

Separate each patient's record into sections of progress/clinical notes, lab results, insurance/billing information, referral/other physician contact and consent information, and miscellaneous.

Use a two-hole punch to create holes in the top of each page and divider. Place the pages onto the prongs in the chart, and place a divider on top of each section of pages.

Label each divider tab to show which section of paperwork is under each divider. Printable or writable labels will work fine.

Place a label for each patient on the edge of the charts. Include the patient's ID number or Social Security number for easy referencing.

File your charts in a file cabinet drawer, in alphabetical order by the patient's last name.
