Complete the application form by filling out your personal information, driver's license number, and vehicle information. State your disability and your official diagnosis, and state whether your disability is temporary or permanent. Provide your physician's name and contact information, as well. You may need to complete an accompanying form giving permission for an agent to speak with your doctor about your disability to confirm the information on your application.
