How to Apply for Social Security Cards for Infants
When a baby is born, getting a Social Security number for the infant is the last thing on some parents' minds. Parents are not legally required to do so until the child reaches the age of one. They can apply for a Social Security card for their infant either before leaving the hospital after birth or by visiting a Social Security office at a later date. There are no fees associated with obtaining a Social Security card.

- Infant's birth certificate
- Parents' Social Security numbers
- Social Security Form SS-5
- Proof of infant's citizenship, identity and age
- Proof of parent's identity

Apply at the hospital, after you have given birth and are providing information for the infant's birth certificate. Check "YES" when asked whether you wish to apply for a Social Security card for the baby. Provide your Social Security number, and if possible, your spouse's.

Apply at a local Social Security office if you don't wish to do so at the hospital immediately following the birth.

Complete Form SS-5 (Application For A Social Security Card). Provide original documents that prove the infant's identity, age and U.S. citizenship. The birth certificate is the preferred proof of age. If you're unable to provide the birth certificate, use another document, for example the child's passport. The proof of identity must include the infant's name, identifying information, and ideally, a photograph. If you're unable to provide these documents, the Social Security Administration may accept the infant's adoption decree, medical records, religious records, school records or school identification card. If using a picture ID as proof of the infant's identity, for example a school identification card, the child might need to be present.

Include a document that proves your identity. A U.S. driver's license, non-driver identification card or U.S. passport is ideal. If you're unable to supply these documents, use an employee, school or U.S. military ID card, health insurance card (not Medicare), life insurance policy or marriage document.

Keep in mind that any documents you provide must be original copies or copies certified by the issuing agency. You cannot use photocopies or notarized copies.

Submit the required information and documents.

Receive the infant's Social Security card by mail. The Social Security Administration will send it after it verifies the provided information and documents. Keep in mind that the post offices of some localities will not deliver the card if the infant's name does not appear on the mailbox.
