Go to the custom shop at the NBA store website at http://store.nba.com/category/index.jsp?categoryId=3358354, as of 2009. You should see a list of the team jerseys. They start with the number "00" before they are customized.

Select the team and type of jersey you want to customize. You can choose to customize an authentic jersey or a replica. Authentic jerseys are of higher quality but are much more expensive.

Enter the unique name you want to appear on the back of your jersey. This can be your last name or a nickname. Then, select your number, anything up to 99.

Select your custom jersey size and click "Preview" to view the jersey before completing the order. Click "Add to Cart" when you are satisfied with your jersey.

Click "Proceed to secure checkout" and enter your billing and shipping information. Place the order.
