  1. Step 1

    Choose or create an employee satisfaction survey.
    Download an employee satisfaction survey online or create your own; just make sure that the questionnaire includes all of the areas you are attempting to assess.

  2. Step 2

    Hand the employee satisfaction survey form out to all employees.

on 10/29/2009 Step 4 is especially important. It only makes sense to do a survey if you plan to value the feedback and make improvements.
