How to Make a Corrective Action Form

A corrective action form is a useful tool for any human resources department. A corrective action form allows an employer to document an employees workplace offenses, as well as lay a clear plan of action for both improvement, and repercussion should the offense happen again.

Instructions

1. Create a field for the employee information.
   Name, job title, and supervisor should all be fields o every corrective action form. The date should be documented, as well.

2. Document the reason for the necessity of a corrective action.
   Create a text field for why the employee performance was either unsatisfactory or against regulation. This should be in as much detail as possible.

3. Make a text field on the corrective action form under the subheading "required improvement".
   This section of the corrective action form allows documentation of how you expect employee performance to improve, as well as a reevaluation date.

4. Create a signature line for employee evaluation coordinator, employee, and supervisor.
   All relevant signatures must be on the corrective action form to make the employee performance tool valid. Having all signatures on the form denotes that all parties are in understanding of the situation and what corrective action will be taken.
