How to Do a Legal Name Change After a Divorce
Making a legal name change is easy following a divorce. Most states will allow you to make a request on your petition for divorce for the judge to restore your maiden name in the divorce decree. This decree represents a legal order to change your name, and you may use the order to update your information at other offices and agencies.

Make sure your change of name is stated in the divorce decree. Include a request in your divorce petition or ask the judge during the hearing.

Make several copies of your divorce decree. You will need these as proof of a legal name change.

Request a new Social Security card reflecting the name change. Call the Social Security Administration at (800) 772-1213 to obtain forms and instructions. Be prepared to send a copy of your divorce decree.

Visit your local Department of Motor Vehicles office to request a new driver's license. Take a copy of your new Social Security card and divorce decree.

Update financial account information: bank accounts, certificates of deposit, personal checks, debit cards, credit cards and 401K accounts.

Make a note on bills as they come in that you have changed your name and would like future statements to reflect the change. Some companies may require proof of a name change or request that you make the change in person. Many companies will accept a written statement with proof of a name change.

Update vehicle information with your name change: car insurance, car registration and auto loan.

Obtain a new voter registration card.

Update medical information: doctor's offices, medical insurance and dental insurance.

Tips & Warnings
Changing your name on some accounts may not be worth the hassle. A certain cellular phone company (US Cellular) failed to change my name on documents after several attempts were made. Simply wait until a new service is found and / or the contract expires.
My name change still does not appear on mortgage or home owners insurance.
Change your name on your social security first. Then, use the change of name on your social security card to obtain a new drivers license.
