How to Replace a United States Passport

A passport is a government issued form of identification stating the holder's nationality and personal information. It is required in order to travel abroad. The following constitutes reasons for replacing a U.S. passport: passport was issued when you were under 16 years old; lost, stolen or damaged passport; previous passport was issued more than 15 years ago; or your name has been changed legally.

Any valid lost or stolen passport must be reported to the U.S. Department of State before a replacement process begins. Form DS-64 must be filled out and submitted to the U.S. Department of State. It can be obtained from the U.S. Department of State website or at an Acceptance Facility or Passport Agency.

Things You'll Need:

  • Passport application form DS-11
  • Proof of citizenship
  • Proof of identity
  • 2 passport photos
  • Fee payment of $75 for renewal
  1. Step 1

    Download and completely fill out Form DS-11. Application can be found online from travel.state.gov. Be sure to write legibly and in black ink.

  2. Step 2

    Obtain evidence of U.S. citizenship, either a photocopy or the actual documentation, by providing one of the following identifications: previously issued, undamaged passport; birth certificate; consular report of birth abroad; naturalization certificate; or certificate of citizenship.

  3. Step 3

    Obtain one form of the following identifications: previously issued, undamaged passport; naturalization certificate; valid driver's license; current government ID; or current military ID.

  4. Step 4

    Have two, identical, color passport photos taken at a drug store or portrait studio. Photos must be 2 x 2 inches in size and must have been taken within 6 months of the replacement request. Additionally, it must be a photo of a forward facing, full face with normal street attire and against a white or off-white background.

  5. Step 5

    Bring the application, DS-11, all necessary forms of documented identifications, photos and a check, cash or credit card in person to an Acceptance Facility or Passport Agency.

Tips & Warnings
  • For an additional fee of $60, renewal processing can be expedited in two to three weeks. Also, various expedited services are available and a replacement passport can be delivered in 24 hours. A request for a larger 48 page passport is available with a signed request at no additional cost.
  • Generally, the normal process time can take anywhere from four to six weeks and could possibly take longer during peak travel times. Be sure to apply for a replacement early in advance.
