Step 1
Locate the American Citizen Services office of the U.S. Embassy or Consulate in your jurisdiction. There are facilities in every major province and city in Canada. Contact information is available in the government section of your phone book, or through an online search engine like Embassies Abroad.

Step 2
Print and complete Form DS-11, the application for a passport, from the U.S. Embassy of Canada Web site but don't sign it yet. Passport applications must be submitted in person.

Provide any of the following documents as proof of your U.S. Citizenship: an original copy of your birth certificate certified by the state in which you were born, a Certificate of Naturalization, Certificate of Citizenship, Consular Report of Birth Abroad, or a U.S. passport issued more than fifteen years ago.

Step 4
Submit proof of your legal status in Canada by providing one of the following: a Canadian Landed Immigrant document, Minister's Permit, Employment or Student Authorization, Report of Birth Abroad of a Canadian citizen, visitor's visa, or Certificate of Canadian Citizenship.

Step 5
Present photo identification in the form of a driver's license, Canadian passport, or student I.D. You will also need to submit two identical color passport photographs 5-by-5 cm in size, taken recently and against a white background. Most consular offices will have a photo service available for a small fee.

Step 6
Return your application and passport materials to the designated consulate and pay the non-refundable fee. The cost is U.S. $100, payable by cash, money order, or credit card. Provide a stamped, self-addressed envelope to have your passport delivered by mail. Otherwise expect up to four weeks processing time, depending on the seasonal workload.
