Instructions

1. Obtain Kentucky DMV form TC96-16. Obtain the form from any County Clerk's Office or online from the Kentucky Transportation Cabinet Website (link provided in Resources section of this article).

2. Complete all applicable sections of form TC96-16. You will need to check one of the boxes and write your initials next to the box under the reason section to release the information. This is important since Kentucky driver's information is protected under the Driver's Privacy Protection Act Section 2721. Sign the form in the presence of a Notary and have them fill out the Notary section.

3. Write a check or money order in the amount of $2.00 in the name of the Kentucky State Treasurer.

4. Mail the form and the check or money order to the following address:
