How to Apply for Lost Social Security Card

According to the Social Security Administration, you are not required to have a Social Security card as long as you know your Social Security number. However, it is useful to keep your card on hand. If you've lost your card and need another, there is no free for a replacement. You are limited to three replacements in one year or 10 lifetime (excluding required replacements because of events such as changes in marital or citizenship status). You can apply for a replacement Social Security card in person or by mail in most cases.

      Obtain and fill out Form SS-5 (Application for Social Security card). The SS-5 is available online from the SSA website (link below). You must print it on white 8-1/2 by 11-inch paper (if you are outside the U.S., 8 1/4 by 11.7 inches is acceptable). You may also get a form by going to your local Social Security office or by calling toll free (800) 772-1213 or for TTY (800) 325-0778. Use these numbers to get answers to any questions you have as well.

      Have proof of your identity. You must provide the original or a certified copy of required documents (these will be returned to you). A driver's license or state ID is accepted as is a passport or military ID. Permissible alternative documents include an employee or school ID and health insurance or life insurance identification. An adoption decree is also acceptable.

      Make sure you have any other required documents. If you are applying for a lost Social Security card for a child you must have identification for yourself and the child plus proof of your relationship. Citizens born outside the U.S. must provide proof of citizenship (if not already on file with the SSA). Non-citizens must show proof they are in the country legally and authorized to work.

      Locate your nearest Social Security office or card center. In most areas you can mail your application or go in person to your local Social Security office. In some areas, you must go in person to a Social Security Card Center. As of October 2009, the areas served by card centers were Orlando, Las Vegas, Sacramento County, Calif., Phoenix and metropolitan New York City. Contact information for each is available online or by calling the Social Security Administration.
