How to Obtain a Death Certificate in Missouri
If the person whose death certificate you are looking for died in Missouri, you have several options for locating the death certificate. You need the name of the deceased and the date of death and will pay a small fee. As of 2009, there was no way to order a Missouri death certificate online.

Making a Request in Person

Download an application to request the death certificate (see Resources below).

Fill out the application.

Bring the application and the fee of $13 to your local health department.

Find the vital records department of your local health department and request the death certificate. You will need to know the name of the deceased and the date of his death.

Print out an application for obtaining the death certificate (see Resources below).

Fill out the form and put it your envelope along with a check or money order for $13 (or more if you need extra copies). On the form you will need to supply the decease's name and date of death, plus your name and address.

Put your address on the second envelope. Your address should go in the "receiver position" not the "sender's." Add stamps to the return envelope.

Insert the self-addressed envelope into the envelope you are mailing to the Department of Health. Folding the envelope to make it fit will be fine.

Address your envelope to: Missouri Department of Health and Senior Services
Bureau of Vital Records, P.O. Box 570, Jefferson City, MO 65102. Add stamps and pop the envelope into the mail. It will take two to four weeks to receive the requested death certificates.
