      Gather the required documentation for changing the name on your passport. If the name change is due to marriage, you need a copy of your marriage certificate. If the name change is due to another reason, you must submit a copy of the court documents legalizing your name change.

      Visit a professional photographer to have your new passport photos made. You must submit new passport photos when changing the name on your passport. See the Resources section below for the latest passport photo requirements.

      Fill out form DS-5504 (name or data change) if your passport was issued within the past 12 months. If your passport was issued prior to 12 months ago, fill out form DS-82 (new passport). Both forms are available for download in the passport section at the Department of State website (see Resources).

      Mail the form to the address listed on the specific form. If using form DS-5504, you will not be required to pay a fee. If using form DS-82, include a check or money order made out to the Department of State. Check their website for the most recent passport fees. You must include your passport with your application package, regardless of which form you are using.
