Step 1
You really need to start on this before you ever think about asking for more money. Remember that businesses are in business to make money, and bosses are there to ensure every effort is either to make more or spend less. Keep a log of your activities and how they impact the organization, useing real numbers to make an impression. Saying "I worked hard on 3 projects", doesn't have the same flare as "my work on the X,Y, and Z projects netted the company $XXXXXXX".

Step 2
Justify why you feel you deserve the raise. Remember that your work ethic and results will be your best friend here. Every dime that your company spends on you needs to be an investment for them. If you are good at what you do, they already know what it would cost to turn your position and have to train someone new.

Step 3
Don't get involved in office politics or bickering. Even with the most shining record, you might not get a raise if you've been rumormongering in the halls about your coworkers and such. That smacks of unprofessionalism, and unprofessionalism never earned anyone a dime (I do not consider reality t.v. as a legitimate source of income).
