Get a passport replacement by using this step by step guide. This article will show you how to replace your passport whether it was lost or stole.

Instructions
  1. Step 1

    First, report your passport as lost or stolen. To do this, you need to fill out a form, Form DS-64 (see resources below) and send it to the State Department's passport services branch, specifically the Lost/Stolen Passport Section.

  2. Step 2

    Next, complete an application for a passport, Form DS-11 (see Resources Below). However, do not sign the application until you are at the passport agency.

  3. Step 3

    Get two passport photos to bring with you to the passport agency. These can often be taken at convenience stores quickly. Make sure you get the proper size requirements for the size of the passport photos.

  4. Step 4

    Take your completed passport application IN PERSON to a Passport Facility. Do not sign your form until you are in person in front of a passport officer. Also, make sure you have the proper documentation with you such as a birth certificate, photo ids, etc, along with the application fee and other documents.
