Planning. Make a schedule and try to stick with it as much as possible. If you eat and exercise at roughly the same time everyday, you will be more apt to stick with it.

Grocery shopping. Make a shopping list of the ingredients you will need for your upcoming meals and stick to it. You should know what meals you will be having before you go shopping.

Frequent meals. You should be eating every 3 hours. If you supply your body with fuel every 3 hours, your body will adapt to knowing it will receive nutrients every 3 hours and will not store anything, such as fat. Calculate the number of calories needed per day using a calorie calculator online.

Avoid junk food & drinks. In between meals do not eat or drink junk food. Do not even have any junk food or drinks in your residence, if it's there you will eat or drink it. Try to consume water with all your meals.

Exercise. You must exercise at least 30 minutes a day if possible or every other day if not. It doesn't matter what type of exercise you do as long as you raise your heart rate for 30 minutes. Always consult a doctor before starting an exercise regimen. Always stretch, warmup,and cool down.
