      The opening of a power of attorney document should have the names and social security numbers of the persons whom will use the document: Example: I, Joan A. Doe, a resident of 222 McDonald Lane Atlanta, GA 35976 ; Social Security Number 411-11-111 designate John B. Doe, presently residing at 1234 Harper Lane Atlanta, GA 35976, as my attorney in fact (referred to as "the Agent") on the following terms and conditions:

      Next the body of the POA should state which terms the person is allowed to make decisions on your behalf. Such as: i)Collect and Manage, ii)Buy and Sell, iii)Borrow, etc.

      The body should state any restrictions or revocations. If it is for a specific transaction, time limit, etc. it should be stated.

      The Durable Power of Attorney should then be signed by the person granting the power, two witnesses, and a Notary. The seal should then be placed on the document and copies given to each person.
