Decide on the type of form you want to create and what information you want to collect with your form.

Go to www.jotform.com and create an account, and then login to your account.

Click on the "New" icon at the top left hand side of the screen. This will open the form wizard.

Select the type of form you want to create by clicking on it.

Select the style of form you want by click on the arrow on the drop down menu on the top right hand side. This gives you the opportunity to select a color scheme and style that will be compatible with your website.

Click on "Finish" when after you choose your style, then save the form by clicking on the Save icon at the top of the screen.

Click on the "Share" icon at the top of the screen. This will take you to a new window that gives you the HTML code that will allow you to embed the form on your website.

Login in to your web host and click on the File Manager. Go to the page you want to add the HTML for your form and add it where you want it on your web page. Save the page.

Login to your Jot Form account periodically to check the data you are collecting.
