Step 1
Fill out the application. You can request an application via phone, mail, in person or online at www.ors.utah.gov. If you would like to request one by phone or by mail it is still best to visit their website to get up-to-date information.

Step 2
Turn in the application. You can return it through the mail or in person with a copy of your divorce decree. If there have been any legal modifications you must include those as well. If your child was not born in Utah you will need to provide your child’s birth certificate. If there was a question of paternity and you have documentation, you will need to submit that. If the child has a deceased parent you will need to include that parent’s death certificate.

Step 3
Once you turn in the application you will receive a call from ORS to go over the application with you. If you return it in person you may have someone go over it with you in person. They look to see if you have filled it out correctly. There are very important pieces that must be filled out like your children’s social security numbers, information about your place of employment and your ex-spouses etc... If there is back child support to be paid you must know how much was paid (if any) and what month and year it was paid, however, you do not need to show documentation of it.

Step 4
Someone for ORS will call you within 20 days of filing to go over the next steps in your particular case. If there is anything you left out of the application or anything you need to modify this is a good time to do it.
