Changing a child's surname in Oklahoma is a relatively easy and inexpensive process. The parent, custodian or guardian of the child must file a Petition for Change of Name of Minor with the court in the county in which the child resides, and pay the filing fees.

The petitioner(s) then must affirm the child's identity and the truth of the statements made in the petition.
