Step 1
Create a new folder in which to save a new form in Outlook 2002 by clicking the drop-down arrow beside "New" on the toolbar and selecting "Folder." Name the folder and select the type of form you will be creating in the "Folder Contains" list. Click "OK" to save the folder.

Go to the "Tools" menu, point to "Forms" and select "Design a Form." Click on the type of form you want to create and then click "Open."

Move a field by clicking the outline of the field on the form design and dragging it to the desired location. Delete a field from the form by clicking the field to select it and pressing the "Delete" key. Add a new field by dragging it from the "Field Chooser" box onto the form.

Publish the new form when it is finished. Go to the "Tools" menu on the form's toolbar, pointing to "Forms" and selecting "Publish Form." Click the "Browse" button and locate the new folder created in Step 1. Enter a name for the form and click "Publish."

Set the form as default for the folder. Right-click the new folder containing the form and choose "Properties." Go to the "General" tab and in the "When Posting to This Folder" drop-down list, select the new form. Click "OK" to apply the changes.
