Apply for disability at the website http://www.socialsecurity.gov. If you prefer not to use the Internet, you can call (800) 772-1212 to set up an appointment with your local Social Security office. If you are hard of hearing or deaf, you can call (800) 325-0778.

Fill out the disability report application (see "References" below). The more information that you have ready for the Social Security office, the more likely that you can reduce waiting times, which can be lengthy. This form can be printed and dropped off at your local agency office or filled out online.

Assemble the necessary information needed. You will need your Social Security number, baptismal or birth certificate and a summary of all places that you worked and what you did at each job. You will need the latest copy of a W-2 or your income tax return for the past year if you were self-employed. Supply the names of all hospitals, caseworkers and doctors who have treated you as well as the dates of your visits. You will need copies of all clinic, caseworker, therapists and doctor reports that you have in your possession, plus any test results and laboratory reports. Also, supply a list of all medications and the dosages that you are taking.

Submit all documents and the disability report to the agency for review. There will be more paperwork to complete as the process progresses.
