Job evaluations are more than an annual necessity. Although sometimes time-consuming, the outcome can be enriching for the employee, his supervisor as well as the organization. Evaluations vary in content based on the needs of the organization, but here are some simple suggestions to get you started.

      Create a header that will include the employee's name, her job title, her supervisor's name and the date range that is being covered in the evaluation.

      Create a section for "Objectives." The three to five key objectives of the employee's job should be listed here.

      Insert a section next to each objective where a rating can be entered to indicate if the employee's performance of these tasks was Satisfactory (S) or Unsatisfactory (U).

      Create a section for Successes and Challenges. This is where the successes and challenges related to the objectives should be listed.

      Create a section for commentary. This is where both the employee and his supervisor can write a narrative about the employee's job performance if they choose to. This section should also include a space for the employee and his supervisor to sign.

  • Provide enough space for items to be added where necessary. Advise the employee and her supervisor that they can attach an addendum for extra commentary.

  • A job performance evaluation should focus on the priorities of the job, although the employee is often responsible for a variety of tasks.
