1. Getting Started

   To start off it is important to know that it may take up to 30 days to recieve your replacement Medicare card. If you need proof of insurance within this span of time , stop in to your local Social Security office and request a proof of insurance letter.

2. Phone In Your Request

   Requesting a replacement Medicare card over the phone is another convenient way to get your card . Call 1-800-722-1213 , provide the case worker with all required information and a replacement card will be mailed to you.

   Note : Generally any method you use to request a replacement Medicare card will take about 30 days.

3. Requesting Over the Internet

   You can request a replacement Medicare card online by logging on to socialsecurity.gov . Once you are on the website, click on the Medicare tab and then select "Replace your lost stolen or damaged Medicare card" . Fill out and submit the online application and wait for your card to arrive in the mail.

4. Request Your Replacement Card in Person

   If you prefer to put in the request for the replacement Medicare card in person , go to your local Social Security office and speak to a case worker . After you fill out the application , turn it in , a replacement card will be mailed out to your home address.
