Step 1
Make sure you can afford to request your Oregon Birth certificate at this time. Fees expected from the vital records office varies in cost from $20 to $32.50 for a first copy and up to another $15 for additional copies. This payment is required as either a credit card, check or money order payment before your order can or will be processed.

Step 2
Gather the information that is going to be required in order to get a copy of your Oregon birth certificate. That information includes:
Full name as listed on the original record
Date of Birth
City and/or County requester was born in
Full name of Father as listed on record
Mothers Full Maiden Name
Current Personal Information: Name, address and Phone number
Copy of requesters I.D. or License
Legal Documentation showing authority to request a copy of the Birth Certificate if the requester is not the person on the record. (Power of Attorney paperwork, signed permission from the person on the birth certificate that has been legally notarized etc.)
Credit card number if ordering online or via fax -
Step 3
Mail in the information fee and request to:

Step 4
Call the vital records office (971) 673-1190. You can place your order over the phone then fax or mail in any required information.

Step 5
Fax a request to (503) 234-8417 including all the information shown in step 2 to get a copy of your Oregon Birth Certificate.

Step 6
Log onto the Oregon Vital records site Vitalchek.com and submit your request for a copy of the Birth Certificate right online by filling out the order form and supplying copies of all requested information.
