      Download the Form I-90 from the Internet (See Reference), which can be filled in online and printed, or you can print it out and use a black ink pen to complete the form.

      Call (800) 870-3676 and request a Form I-90 by mail. The USCIS will send you a form in the mail that will need to be completed and returned.

      Contact the Port of Entry, USCIS Office or American Consulate nearest you if you are out of the United States and need to replace your card. You must do this before you can file a Form I-90.
