Birth certificates are necessary for many application processes in Alabama: obtaining a passport, a marriage license or a learner's permit.       Fill out the application with as much information as possible.

      Write a check or get a money order for $12 for one certified copy.

      Mail the application and the necessary money to: Alabama Vital Records, P.O. Box 5625, Montgomery, AL 36103-5625.

      Go to any county health department in Alabama.

      Request a certified birth certificate and any additional copies you want.

      Present required information. That information includes as much of the following as possible: full name of person on the certificate, date of birth, gender, county or city of birth, hospital of birth, full maiden name of mother, full name of father, your name, your signature, your relationship to the person whose certificate you are requesting, address where the certificate is to be mailed and a your daytime contact phone number.

      Fill out a check or money order for $12 for a certified copy of the birth certificate.
