How to Fill Out an I-134 Affidavit of Support
Knowing how to fill out an I-134 Affidavit of Support form is important if you want it to be approved by the Department of Homeland Security. The Affidavit of Support is used to clarify that you will support an alien who enters the United States, so this person does not have to rely on public assistance. As a sponsor, you must have enough income or assets to ensure that you can and will support this person while she is in the country.

Read pages 1 and 2 of Form I-134 so you understand what your responsibilities are and what is expected of you. This step is important because you will have to affirm that you have read and understand the responsibilities at the end of the Affidavit of Support form.

Place your name, address, city, state, zip code and country at the top of the form. Print or type this information.

Start with question 1, which asks the date of your birth and what city and country you were born in. Answer a, b, c and d only if you are not a native U.S citizen born in the country.

Answer the next questions, which ask for your age and how long you have lived in the United States.

Read and answer the next question, which requests information about the person you are sponsoring. You must enter the person's entire name, and any other dependents or a spouse who will join the person on his entry to the country. List his current address, marital status, age and gender, and the relationship he has to you.

Read lines 4, 5 and 6 before continuing to line 7. On line 7, you will enter your type of business and address of the business. You will have to divulge how you make money, how much value you have in personal property, and the value of your life insurance, bank accounts, stocks and savings account. Enter the amount of any mortgage or any other real estate you may have, and where the property is located.

Enter the names of people who are dependent on you, such as children or a spouse. Answer question 9 only if you have submitted previous affidavits, and state whom it was for. Answer whether or not you have asked for a Visa for any persons, and give their names.

Check whether or not you intend to make contributions to support this person. If you are to support this person, give details pertaining to how long you will do this.

Visit a notary to sign the oath of affirmation. It must be witnessed.
