CHECK YOUR CONTACT LENS BOX.
If you still have an old contact lens box, you should be able to find your contact lens prescription. You'll need the brand, base curve, power, and diameter. If you're unsure which number which, just ask a customer service rep at the contact lens store to help. -

GET IT FROM YOUR OPTOMETRIST.
Once you have been fitted for contacts by your optometrist, you can request your contact lens prescription. Sometimes, they'll refuse to give this to you or try to make you pay for it because they want you to purchase from them. Be persistent. They are required by federal law to give you your contact lens prescription. -

REQUEST THE HELP OF A REPRESENTATIVE AT THE ONLINE CONTACT LENS STORE.
Larger contact lens stores such as 1-800-CONTACTS (link with coupon code in resources section below) are able to locate your optometrist and are willing to call and help get your prescription information.
