Step 1
Unless you plan to only download free items, you first need to purchase microsoft points online at: http://marketplace.xbox.com/
These microsoft points are the currency used in the Xbox Live Marketplace. -
Step 2
Use your Xbox Live username and password to login in the upper right hand corner of the page. Your point balance should be displayed in the upper right hand corner with an "add" button. Click it. If you can't find that add button, click the one on this page:

Step 3
At the purchase page, choose the number of points you want. I recommend first adding 1000 points if you plan to purchase downloadable games which usually cost around 800-1200 points. Enter your payment information when prompted and confirm purchase.

Now that you've got some points, browse the Xbox Live marketplace online at:
http://marketplace.xbox.com/en-US/games/default.htm
or turn on your Xbox360 console and shop under the marketplace tab. Be sure to log in under the same username that you used to purchase microsoft points online for your points to show up on your Xbox360. -
Step 5
Once you've found something you want to download, select download to Xbox360 if you're shopping online or download if you're shopping on your Xbox360. Now just wait for the download to complete and play. The games will appear your games library. I recommend Castle Crashers, Braid, and Super Puzzle Fighter II Turbo as some great games only available for purchase online.
