If you were born after 1910 in the United States, there's a good chance the state of your birth has a record of your birth in its vital records department. You will need to follow the directions for your state to get a copy of the birth certificate. Most states require two forms of identification, including a Social Security card.

      Visit the government's Centers for Disease Control website. The CDC site has a link to the vital records for every state as well as the city of New York. American Samoa, Guam, Puerto Rico, the Northern Mariana Islands, the Canal Zone and the District of Columbia.

      Click on the link to the state of your birth.

      Click on the birth link on the left side of the new page. The birth link explains what that location requires to get a birth certificate, including the price. For example, in 2011 New Mexico required $10 for a copy while Kansas asked for $15.

      If you prefer to pick up a replacement birth certificate in person, the CDC site has the address for the state or local office that handles such requests.
