Once you have made contact with a plan, their representatives will try to determine whether or not your kid(s) are eligible for New York Child Health Plus health insurance. If so, set up an appointment with a representative. He or she should inform you as to what paperwork is required and what documents you will need to provide.

Make photocopies of the documents that are required for New York Child Health Plus health insurance. Typically this includes items such as proof of identity for yourself and your child (as well as a spouse, if applicable), proof of income (such as four consecutive pay stubs) and proof of residency (such as a handwritten envelope addressed to you with a date stamp from the post office). If you qualified for a low cost plan you might need a check for a monthly premium. There can be other requirements so be sure to consult your plan first about this. Make each copy on a separate piece of paper.

Step 4
Meet with the representative. Answer his or her questions and fill out the paperwork for your kid or kids. Select a pediatrician or doctor from the network of the plan. The meeting should take about an hour.

Step 5
Once you have finished the application for New York Child Health Plus health insurance it will be sent to the State. If and when it is approved, you will be contacted. Congratulations, your kid(s) are now covered through New York Child Health Plus health insurance.
