Anytime you fill out forms 5498, 3922, 3921, 1099, 1098 and W-2G, you must submit a copy of each with Form 1096 to the IRS by the deadline. You must know how to fill out a Form 1096 so that you have all of the information correct before sending to the government.

Instructions

      Enter your name or business name as the filer in the top box. Include your address, city, state and ZIP code.

      Enter the name of the person to contact if the IRS has any questions in the box just under the box that contains your name and address information. Enter an email address if you have one and your telephone number as well as a fax number.

      Enter your Employer Identification Number on line 1. Enter your Social Security number on line 2.

      Enter the total number of forms that you are sending on line 3. On line 4, enter the total amount of federal tax withheld from all of the forms you are sending. On line 5, enter the total amount entered on each form.

      Place an 'X" in the box that represents which type of form you are filing. Look closely at each selection and compare it to the form number you are filing. In box 7, place an "X" only if this is your last return. This is meant for businesses that are no longer in operation.

      Sign and date the form before placing in the envelope and send to the Department of Revenue.

Tips & Warnings

  • If you have more than 250 forms to send, you must use the online transmission service.
