How to Create a Form in Word

If you want to create easy forms, you will want to know how to create a form in Word.

Word has many formatting options that you can use to create forms, even without advanced knowledge of Word. You can create a form in Word to be used as an order form, business form, survey, registration form and more.

Below are steps to help you create a form in Word which you can then print out and give to customers, students or other contacts to fill out.

Note: The following instructions are intended for beginner Word users. There are also advanced form functionalities for advanced users which are not included in this quick guide.

      Create your form text -

      Before adding in form fields, decide on all the text you will need on your form. Also decide on which input fields you will need filled in.

      Use blank underlined space for text input -

      The simplest way to create forms is to simply create a blank space for users to input their free text. To do this, simply press shift together with the dash key (the key located to the right of the zero). Make sure to leave enough space for user to input the free text.

      Use tables as input fields -

      Next, create a table with 2 columns and as many rows as needed for all your input fields. The first column will hold the name of the field you wish to input and the second field will be empty space for the user to input their text.

      For example, if you have fields "name", "city" and "state", you will create a table with 2 columns and 3 rows (total of 6 cells).
      The left hand columns will read:
      Name:
      City:
      State:

      The columns on the right will be empty for users to input their information.

      Note: Make sure to put borders around the cells so that there will be empty boxes as input fields.

      Create check boxes

      To create a check box input field do the following:
      - Write your desired text
      - Place your cursor on the place where you would like to insert the check box
      - Open the forms tool bar
      - Click on the check box icon - this will insert a check box in your desired spot

      Print out the form and give it to clients, customers, students or other users to fill out.

Tips & Warnings

  • Make sure to leave enough free space in free text fields for users to input information. Be especially careful if these forms will be filled in by children.
