Some of the ways the data can be collected incdlue surveys, from sources that include clients, other members of the department in question or supervisors.
-
Step 2

Analyze the candidate's degree of success in meeting each goal in relation to relevant professional standards. Use the collected data to support your statements.
-
Step 4
Indicate at the end of each section whether the candidate has made no progress, some progress or significant progress in each area. State whether he has marginally, mostly or fully accomplished the given goal---if at all.
-
Step 5
Recommend a course of action that will help the candidate make more progress and/or set new or adjusted personal goals.
-
Step 6
Include a "Self-evaluation" section at the end of the document in which the candidate can reflect on her performance and progress.
