It is important to provide your new address so insurance information is not delayed. The post office will forward your mail for a few months to your new address, but once the forwarding period expires, you will not receive your insurance information.

Instructions

Change Health Card Address Online

1. Look on the back of your health insurance card for your insurer's email address or website. Visit your insurance carrier's website.

2. Log in to your account information using the detailed information from your health insurance card. Find the "manage account" section and follow the instructions to change your address.

Change of Address Over the Phone

1. Check the back of your health insurance card for the insurance carrier's telephone contact numbers.

2. Call your insurance carrier using the information or billing services contact number.

3. Speak with a customer service representative to change your health insurance card address. The representative will need proof of your identification and your current health card account number.

Change of Address through the Mail

1. Locate the insurance company's mailing address on the back of your health insurance card.

2. Write a letter to request a change of address on your health insurance card. The letter should contain your previous address, your new address, your health care account numbers and your new telephone number.
