Of course we all try to keep our important documents protected and safe. But sometimes, life happens and we are suck trying to replace important information. If you are or had a child born abroad, you may be stuck trying to find proof of birth information. Follow these instructions on requesting a Consular Report of Birth Abroad of a Citizen of the U.S.A. (FS-240).

Instructions

Things You'll Need:

  • birth information
  • FS-240 number
  • passport number
  • notarized affidavit
  • $$ for State Department fee
  1. Step 1

    To request a copy of a Consular Report of Birth Abroad, you must submit a written request to the following address:
    Department of State
    Passport Services
    Correspondence Branch
    1111 19th Street, NW, Suite 510
    Washington, DC 20522-1705

  2. Step 2

    In the request, include the full name of the child at birth, plus any adoptive names; date and place of birth; names of parents; serial number of the original FS-240 (if issued after November 1, 1990, and if available); and any passport information available.

  3. Step 3

    The request must be signed by the requestor. Additional, a notarized affidavit explaining the circumstances of why a replacement is necessary must accompany the request. Mail the request, the affidavit, and a check or money order of $30 payable to the Department of State, to the address listed in Step 1.

Tips & Warnings
  • These steps are for children born of 2 US citizens, and may not apply to a child that has duel citizenship.
  • Government Fees change over time. Check with the State Department.
  • Keep important documents in a safe, or safe deposit box. Keep copies of information in a secondary safe location.
  • DO NOT scan and store documents on your computer's hard drive.
