How to Apply for Dental Medicaid
If you are having difficulty paying for dental insurance, you may be eligible for dental Medicaid. You will need several pieces of personal information to start the application process.

- Proof of identity (driver's license or Social Security card)
- Proof of residence (copy of a utility or phone bill)
- Proof of income (recent tax return or pay stub from employer)

Review state requirements for eligibility online (see Resources) or visit or call your local health and human services office to determine if you qualify for dental Medicaid.

After you have been deemed eligible, submit your application to your local health and human services office. Typically, that office will request various forms of identification, such as your driver's license and/or Social Security card, as well as proof of residence, which can be shown with a recent utility or telephone bill, and income, for which you can show a recent tax return or pay stub.

If you are approved for dental Medicaid coverage, you will receive your coverage card in the mail. Present this card to any dentist that accepts Medicaid.
