• Death certificate

Instructions

1. Gather all the information that is needed to request a copy of a deceased person's US passport. You will need a copy of the deceased's death certificate, first and last name, address, place of birth, the reason for the passport, your telephone number and a valid ID of the person requesting the copy of the passport.

2. Send a check for the fee of $60 to cover the cost of the copy of the deceased person's U.S. passport. The payment can be made with a personal check or a money order. Make the check out to "Department of State."

3. Place all the above information in a envelope and mail it to the following address:

4. Seal the envelope with the requested information and place a stamp on it. To ensure a faster and safer delivery, send the mail by priority mail.
